I Bandhana Verma W/o Sh. Susheel Kumar R/o Rajpura Tehsil Rajpura
District Samba, Occupation- House-wife, Age- 32 years do hereby solemnly
affirms and declares as under:
1. That the Deponent is the permanent resident of U.T. of J&k.
2. That the Deponent has applied for the direction of this hon’ble court for
issuance of birth certificate of her child namely Mukuand Verma who was
born on 12-02-2018 at home i.e. Village Rajpura Tehsil Rajpura District
Samba.
3. That due to lack of knowledge his birth particulars could not be registered
in the concerned authority till date.
4. That the deponent has many times approached to the concerned authority
to issue the birth certificate of said child but they refused to the same and
told that a direction of this honble court is very much required for the
same.
5. That the birth certificate is required to deponent for taking admission in
school and other necessary documents too.
6. That the deponent has to submit this affidavit to the concerned court as an
evidence.
7. That contents of the affidavit are true and correct and nothing has been
concealed therein.
