Reason of referral: He is not performing well in academics. The client posing conduct and
behavioral issues.
Counseling session :
When the client and school coordinator entered in the counseling room, the counselor
immediately addressed them both. The client was not newbie, he had previously been to the
instructor before for a couple of times.
The coordinator began berating him for his behavior and academic carelessness. All the teacher
have complaints for him.
The counselor quietly listened to coordinator. She sure that the client will complete his work and
will also improve his conduct.
The client was still looking down and anxious. Counselor began by asking him Why are your
teachers having issues about your conduct? What do you do in the class? The client started
speaking „Mam actually our class monitor always complaints about me, even if I done anything.
He does not like me and therefore he bothers me all the time and complaints to teachers don‟t
listen to me and scolds me instead. The counsellor responded “But why don‟t your teachers listen
to you and listen to the monitor?” At this client remained quiet for a while and then said “Mam
because earlier I was naughty but not now and that monitor is good in studies so teachers like
him.” Counsellor smiled at him and stated “So you do understand the perks of being good in
academics.
Taking the session further, Why didn‟t he complete his work? To tis client replied Mam I
wanted to do and I thought I will start tomorrow but couldn‟t do.” The counsellor explained him
the cost of procrastination , following which she recommended him some strategies to overcome
this problem.

Mental State Examination (MSE):

Appearance: - ABC was healthy. He appeared neat and groomed.

Behavior:- In the beginning appeared little hesitant and avoided eye contact in his teacher‟s presence, but later he was confident. He was a reflective boy with discerning abilities.

Speech: - His speech was absolutely fine but was unable to speak his heart out in front of her teacher.

Spot Diagnosis:

It was evident from the session that the client was having issues of procrastination.

My Observation:

ABC was smart and clever. He was reflective and had a good sense of judgement.

Recommendation:

The client who was struggling with procrastination received the most important suggestion. He was unable to finish his work. It was very overwhelming to him. Therefore, the therapist gave him counsel on how to organize his daily list of tasks, time management tools, create a personal timetable.

Plan of Action:

 The counselor proposed meeting with the client once a week to assess his development.  She additionally proposed meeting with his parents to talk about his procrastination problems.
